The backstory revolves around Sawyer
Desmond is thinking
The cages. Danny wakes up Sawyer and Kate and tells them it's time for work. Sawyer says, "Sounding a little stuffy there, Chinatown. Need to blow your nose?" A reference to the face pounding Sawyer gave him last episode. "Just give me an excuse." "Thought I just did. What I gotta do, talk about your mother?" Danny grabs for him. A voice over the radio stops him. The voice says something about them being back, but it's crackly to me. Ben and Juliet come running through the jungle. "She's hurt bad, Juliet." Some Others enter the clearing carrying a stretcher with Colleen on it. A lot of blood is on her hands and she is conscious at that time. Sawyer and Kate watch as she is carried away surrounded by Others.
"What happened?" Kate asks. "We happened," Sawyer replies. "Been on this rock long enough to realize they ain't in the business of shooting each other. We did it. Our team." Kate looks upset. "Are you smiling?" Sawyer tells her he's smiling because they just got their ticket out of there.

The cages. Sawyer is working on the fish biscuit contraption again. Kate asks him what he's doing and Sawyer shushes her. He sets off the contraption, pouring water onto the ground. Sawyer plans to shock the next Other who comes to the cage. Kate tells him seh's impressed. He tells her to wipe the stars from her eyes. She asks about Jack and Sawyer says they don't even know if he's alive, they have to take care of themselves, "It's every man for himself, Freckles."
Backstory. Sawyer is in prison and boxing with another inmate. He knocks him down. VPCI They see Munson being beaten. Rumor has it Munson stole ten million, but no one ever found the money. Sawyer is inmate number 840. Munson is inmate number 248. Sawyer's fellow inmate tells him the warden—and he makes it sound like Sawyer and the warden don't get along—is keeping the man alive.

The cages. Ben approaches Sawyer's cage. Ben asks how much he weighs. "180, give or take." How old are you? "32" "Don't lie." "35" "Good" Ben steps into the puddle that Sawyer has created. Sawyer grabs his arm and kicks the button, expecting to electrocute Ben, but nothing happens. He asks Ben what they did. Ben calmly replies, "We turned it off." Ben hits Sawyer and knocks him away from the cage, then enters and hits him until he knocks him out.
Inside a cell. Sawyer wakes up, strapped to a table. Tom is speaking, "It's been two days since the sky turned purple. We've been blind. Our coms are all down. I can't get them back up again and in case you've forgotten Colleen's in critical..." Ben interrupts and says Juliet's taking care of that. An Other gives Sawyer a stick to bite down on "for the pain". Another Other holds up a very large needle. Sawyer struggles, but he is securely tied down. He yells for them to wait.
Jack's cell. Jack is eating something when the speaker crackles on. He hears Sawyer begging them to wait, stop.
Sawyer's cell. Other A points the needle toward Sawyer's chest. Other B stops him and says, "No, you have to go through the sternum. The sternum. Like in the movie." (The movie is "Pulp Fiction".)Other A says he knows. All the while Sawyer is telling them to wait. Other A stabs him in the chest.

Beach. Paolo is hitting small green fruit into the ocean with the golf clubs. Desmond tells him he wants to take a club. Paolo is wearing a green, white, and red shirt with a logo on it. Maybe some sports-minded person could tell me what it is? Paolo says to take the five iron because he never uses it. That way, he continues, when Desmond dies in the jungle, doing whatever he is doing, Paolo won't have to go looking for it. Desmond seems a bit put off by Paolo's attitude, but thanks him and turns to leave. Paolo swings and Desmond suggests he might want to square his shoulders a bit more. Paolo sounds disbelieving when he asks Desmond if he golfs. Desmond replies, "I'm Scottish." The comeback is reminiscent of season one when Jack gives Kate a golf suggestion and she asks him if he golfs. He replies, "I'm a doctor."
Sawyer's cell. He wakes up again, this time with a big bandage across his chest where the needle must have stuck him. Tom walks in with a cage containing a rabbit with a big number eight painted on its back. Ben walks in after and takes the cage from Tom. He holds it over Sawyer and shakes the rabbit for some time until the rabbit falls over, apparently dead. Ben explains to Sawyer that the rabbit had a pacemaker and when the rabbit's heart reached a certain beat rate, the pacemaker caused the heart to explode. Ben goes on to say that they have done the same thing to Sawyer. When his heart reaches 140 beats per minute, his heart will explode. Ben has done this to keep Sawyer from escaping. He puts a watch on Sawyer that he says will let Sawyer know that his heartrate is getting close. Ben says that if Sawyer tells Kate about any of this, he will put a pacemaker into her, too.
The cages. Sawyer goes back to his cell much calmer, almost like he's broken. Tom and an Other leave bowls of water and clean clothes for them to clean themselves up. Kate asks Sawyer what happened, but he refuses to tell her anything. She begins to clean up and Sawyer glances at her as she does, setting off his watch. He pours the water over himself to cool off.

The cages Kate says she can fit through the top bars. Sawyer tells her not to, he thinks they should chill out and get the lay of the land.
Jack's cell. The speaker crackles and Jack hears, "...everything's under control..." Juliet rushes into the viewing area and Jack demands to see Sawyer. Juliet has blood on her clothes and Jack is sure it's Sawyer's. She tells him it's not and she needs his help.

Area Ops 4. Ben runs up to Juliet and Jack. "Have you lost your mind? You brought him here?" Juliet points out that he's a doctor and she overrules Ben. They scrub in and Jack looks at some X-rays. Juliet says they're not hers and leads him into the room where Colleen is on the table. Jack tells Juliet to send everyone out. Juliet tells him she got the bullet out, but Colleen is still bleeding. Jack finds a place behind the liver where she is bleeding from. Juliet says she isn't a surgeon and Jack says he knows. When Colleen flatlines, Jack asks for the crashcart. Juliet looks at him for a moment and he repeats himself. She tells him it's broken. Nothing like this has happened before. Jack begins chest compressions and we see the monitor drop from 39 to 16, then soon after, Jack stops. He starts to declare time of death, but looking around, he doesn't see a clock and just says, "She's gone."
Danny Pickett, Colleen's husband, has been watching from a viewing room and we hear him yell, "They did this! They did this!" He storms out.
The cages. Danny stomps toward Sawyer's cage with murder on his face. He enters Sawyer's cage and pounds on him. Sawyer flinches and tries to get away, but keeps glancing at his heart rate monitor. Soon, he is too beaten to even do that. Danny yells at Kate, does she love this man? After several more punches, Kate says she does. Danny stops hitting Sawyer. He leaves Sawyer leaning on the outside of Kate's cage.

Prison. Sawyer is reading "Of Mice and Men" by John Steinbeck. Munson comes up to him and says he needs to talk to Sawyer. They walk down a hall and Munson tells him he was right. His wife has hired a private investigator and she will find the money. Sawyer imparts some of his wisdom. "That's why you never get attached. Once you care, that's when they can come at you." Munson asks what Sawyer's woman wanted, referring to Cassidy, and Sawyer doesn't answer. He tells him, "Now you've had a good cry, mind if I go back and see if George gets his farm?" Munson asks Sawyer to move the money. Sawyer refuses and starts to walk away. Munson pleads and Sawyer stops.
The cages. Kate asks Sawyer if he's okay. We hear thunder in the distance. Kate wonders why Danny beat Sawyer and Sawyer asks if anything the Others do makes sense. Kate climbs up and out of the cage. It starts to rain. Kate comes over to Sawyer's cage, upset that Sawyer is so scared. She tries to break the lock on Sawyer's cage. Sawyer tells her to go, leave him. The watch starts to beep and Sawyer glances at it. Kate demands to know why he didn't fight back. Sawyer says if she really loves him, then go. Kate tells him she only said that so that Danny would stop hitting him. It stops raining. Kate gets back into her cage.
Sawyer tells her, "Every man for himself." Kate says, "Live together, die alone." (Echoing Jack's words.)
Monitor room. Ben and Tom are watching them on monitor five. Tom says Danny wants to kill Sawyer. Ben says Danny can wait. Tom asks if he should bring Jack back. Ben considers this a moment, then says he wants Jack to sit with Colleen a while longer.
Area Ops 4. Jack is handcuffed to the operating table where Colleen's body is covered by a sheet. A clearly shaken Juliet walks in and apologizes that they put handcuffs on him. She says she's a fertility doctor and she's not used to death. Jacks asks Colleen's name. Juliet says she should have come to get Jack sooner. Jack says it wouldn't have mattered, there was nothing anyone could have done. Juliet asks if he's just saying that to make her feel better. Jack looks at her for a long moment, then laughs a short laugh. He says he doesn't care about making her feel better. Juliet seems to accept this and Bends to remove the handcuffs. Jack grabs her wrist and asks whose X-rays he had seen. They belong to a man, about forty years old, with a very large tumor on his L4 vertebra. Jack notices the remarkable coincidence that he is a spinal surgeon.

Desmond's experiment
The cages. It's night. Ben wakes up Sawyer and tells him he wants to go for a walk.
Prison. Sawyer is escorted by several guards to a meeting with the warden and two Treasury agents. Agent Freedman asked what he has. Red bronco parked in a storage facility in Sawgrass off 441, in 23C. The last six years of Sawyer's sentence are commuted for this. He'll get his commission once the money has been recovered. Sawyer asks for the money to be deposited in a new account in Albuquerque in the name of Clementine Philips, with no way to trace the money back to him.
Mountainside. Ben leads Sawyer and a few Others up the side of a mountain. Sawyer asks if it's "that place you always wanted, George?" Ben doesn't recognize the reference and Sawyer tells him it's from "Of Mice and Men." The watch starts to beep and Sawyer gets nervous. He asks Ben if he brought him up there to kill him. Ben laughs and tells Sawyer the only thing they put in him was doubt. Ben pulls a rabbit with a number eight on it out of his bag, alive and healthy. He says they gave it a sedative. Sawyer asks how he knows that's the same rabbit. Ben says he doesn't. But that's not what he wanted to show him.
Sawyer stops, then walks forward to the edge of a bluff. He sees another island separated by quite a distance of water. Ben tells him the island he's standing on is twice the size of Alcatraz (Alcatraz is twenty-two acres, making l'Île d'hydre 44 acres). The other island is the one flight 815 crashed on. Sawyer asks if he did all that just to keep him in a cage. Ben says they did all that because the only way to gain a conman's respect is to con him. Then he says that Sawyer is pretty good at it, but they are better. He mocks Sawyer for denying that he cares about Kate. He quotes from "Of Mice and Men" about loneliness and Sawyer doesn't recognize it. I guess he didn't read much of it. Ben tells him he needs to get him back to his cage.
